Help - skin rashes/spots when in sun

Hi everyone, hoping someone can help as my GP is clueless and the waiting list is years for a dermatologist.
For the last 3/4 years, every time I go out in the sun I get a bad rash of spots. It starts out with a few and then more and more, and after time some will join together into a big red raised area.
They start coming up within 20 minutes of me being out in the sun, even when I’m covered, so it must be heat as well that makes it happen.
They are very itchy, and occur all over, but especially badly on my chest and arms. They don’t have heads and I try not to itch them as that is when they seem to join together.
I wear factor 50 sunscreen, sit in the shade and cover up but they still come. I’m definitely not allergic to the sun cream, tried different ones and none at all but same result.
It’s basically making me not want to go outside ever again, or on holiday which is unbearable.
I live in the UK and never had problems with the sun until 3/4 years ago and went on holiday to hot places every year.
Does anyone know what it is/how to stop this happening please?
