Sounds like you have had a significant amount of sun exposure, so it would be a good idea to get them checked. All consultant dermatologists will be able to check your moles, so you can search for one local to you. Or alternatively, see if your GP can help. Good luck!

Sorry to hear you are struggling with this. It’s difficult to know from the photo how active the condition is. Unfortunately, even previous changes of necrobiosis can leave the skin permanently thinner with some redness. I think it’s worth Seeing your Dermatologist again to discuss potential options...

I suggest you go and see your Doctor. More recently, we have been using tablets for melasma, which might be an option for you.
