clearly a complex problem and you have had 2 (3 if you count vasectomy) operations, neither of which have sorted things out.
If there is no clear anatomical abnormality on the scans and examinations then you might not ever find out the true cause 100%.
I would think seeing a pain specialist might be the next step. Probably abnormal pain response from the previous procedures
I certainly wouldn't have any further surgery.
It is probably a case of getting a good plan about how best to manage the discomfort.
